This needs to be completed to reorder your prescription. It should only take a few minutes. This is an opportunity for you to tell us how things have been going with your medication. This form will ask how you feel on your current dose, any side effects and will ask you to check your weight. It will also ask you if you are happy to increase the dose, as per the titration regime.
The completed form will then be reviewed by one of our prescribers and if it is safe and appropriate to do so they will prescribe your next pen. If they have any questions they will call you directly.
